- Medical history you have available
- A list of your current medications
- All scans and X-Rays (preferably on a disc)
- All recent blood test results
- Health insurance details
- Medicare card
- Any other requested tests or paperwork
Please come ten minutes early to complete administration paperwork, or print off the patient details form and bring it with you completed.
